Job Description:

Position Objective:The assessor will complete behavioral health assessments and reassessments for children, adolescents and adults. The Assessor will also complete an Individual Recovery Plan and obtain signed consent documentation along with each assessment/reassessment. This position may be part or full time, and the role may be served in conjunction with another position.The assessor will ensure consumers who enter services receive comprehensive assessments and participate in the development of a person-centered Individual Recovery Plans. Assessment information is relayed to the state and appropriate insurance companies to seek out appropriate authorizations.Essential Duties:1. Complete assessments, reassessments and Individual Recovery Plans.2. Obtain and upload signed consent documentation for each Behavioral Health Assessment via DocuSign or in-person documentation (written or via signature pad).3. Document assessments and contact notes, using a computer based reporting system, i.e. Care Logic.4. Actively participate in team meetings with the Manager.5. Attend monthly Clinical Supervision group unless fully licensed as a LPC, LMFT, or LCSW.6. Actively and timely responds to consumer crisis and supports crisis intervention efforts when BHL is called.7. Collaborate with outside agencies as appropriate, i.e., Probation, DFCS, DJJ, CME, LIPT, School, Court system.8. Maintain a caseload size which will allow for quality services and which will also meet productivity targets.9. Participate in community collaborative meetings.10. Ensure that IRP's are person-centered and meet State Guidelines.11. Provide training to new intake workers on intakes and writing treatment plans if needed by Program Supervisor.12. Play an active role in PQI process development and monitoring.Other Duties:1. Case conferencing with the Treatment Team.2. On an as-needed basis, assist with Walk-in or "On-call" assessments.3. Assist with Hospital Discharge Appointments (if associate or fully licensed).4. Complete Diagnostic Assessments (MUST be fully licensed). -Knowledge, Skills and Abilities Required:1. MUST BE FULLY LICENSED- Georgia Licensure as a Professional Counselor, Clinical Social Worker, Marriage and Family Therapist or Psychologist is preferred. -2. Demonstrated clinical skills, based on training and experience, necessary to assess clients and provide individual and/or group psychotherapy/counseling.3. Knowledge of and practice by the ethical standards of their professional association, State Licensing Board and Georgia HOPE.4. Knowledge of and practice by the confidentiality standards of HIPAA regulations.5. Computer skills adequate for inputting session documentation, billing and scheduling.6. Capacity to function collegially and communicate within a Treatment Team.7. Ability to process cases information and benefit from supervision to assure service is provided at the Clinician's highest level of professional competency and improve professional knowledge and skills.8. -Contribute to Georgia HOPE's culture of safety and inclusion by demonstrating a commitment to learning how to engage with issues of diversity, equity, and inclusion. -Qualifications for this position include:

Master's degree in counseling, Social Work, or a related fieldAssociate or Fully Licensed (LPC, LCSW, LMFT) not required, but must be -license eligible.Supervision -is provided and required.CE opportunities are -available.Basic computer skills, including familiarity with using the internet and Microsoft WordAbility to navigate online electronic medical record system (CareLogic)Ability to work independently and manage time -efficiently.Physical Requirements:Candidates/Incumbents will demonstrate the ability to sit for long periods of time and interact with clients and other staff members. Travel may be required. Routine bending and lifting. -Working Conditions:The Clinician may work in an office setting, but there will be occasions wherein s/he will provide services in the client's home or a community setting. At times, the incumbent will be required to relate to clients and/or their family members, exhibiting intense emotions. The Clinician may provide up to 80% of their services in the community and may work nights and weekends to achieve the necessary level of intervention for rehabilitative support, and to meet productivity targets. -Equipment and Materials Utilized:Computer, computer programs and internet, Agency forms and the APA DSM-IV.Health Connect America is a multi-state, community mental health, behavioral health, family preservation, and therapeutic foster care services provider.
