We look forward to hearing from you Job Description: Assertive Community Treatment (ACT) is designed to engage persons with a Severe Mental Illness (SMI) whose needs are not met by traditional outpatient services. Services are delivered in the context and environment where they are needed (i.e. the team goes to the person). Under administrative direction from Chattanooga Clinic Manager, provides clinical leadership to the ACT Team in assessment, treatment planning, general healthcare, medical and psychosocial approaches. Essential Duties and Responsibilities: Directs day-to-day clinical operations of ACT team Provides direct supervision of team members Schedules staff work hours to ensure appropriate coverage Leads daily team and treatment planning meetings Evaluates status of all consumers and provides appropriate coordination of treatment to meet their challenging needs Directs and coordinates consumer admission and assessment processes Schedules admission interview Supervises, directs, and coordinates completion of comprehensive assessment/reassessment of each consumer Utilizes management tools to help ensure team's philosophy and goals are in accordance with the ACT model Participates in staff recruitment, interviewing, hiring, orientation and performance plans Oversees medical record documentation to ensure quality and accuracy Provides on-call crisis interventions and back up according to team's policy Participates in engaging and motivating team clients in all treatment goals while providing direct service Manages ACT Team in maintaining organization's required productivity standards Oversees completion of all required documentation in timely manner consistent with organization's guidelines. Communicates regularly and attends required meetings with Hamilton County FUSE Leadership Team and ACT Program Director. Compliance requirement for annual completion of Catalytic Coaching and KPI forms Regular and Reliable Attendance is required Other Duties and Responsibilities: All other duties as assigned. Physical Demands: Percentage of Time/Frequency Standing 25 % Squatting 5 % Kneeling 5 % Bending 5 % Sitting 45 % Driving 15 % Lifting up to 20 lbs. Job Qualifications: Master's degree and license eligible in Tennessee in Social Work, Psychology, Counseling or a related field. Thorough knowledge of relevant client population characteristics and related treatment planning. Excellent oral and written communication skills. Good interpersonal skills. Good public speaking abilities. Thorough knowledge of ACT model. Good supervisory skills. Two years' experience supervising an ACT Team. Over five years clinical experience with a supervisory background, and experience with program management, ACT Team management preferred.
