OnTrack Hamilton County Team Leader The Helen Ross McNabb Center ( is a premier not-for-profit provider of behavioral health services. The Center provides quality care and serves all ages in a continuum of prevention, early intervention, social, co-occurring, addiction, outpatient and crisis services. The Center is seeking motivated staff members that possess the ability to relate to individuals and families of varied ethnic and cultural backgrounds, ages, educational levels and economic circumstances with respect and dignity. Excellent verbal, written and computer skills are necessary in order to work effectively and collaboratively with clients, collaborating agencies, and community professionals, as well as, maintaining proper documentation. Clinical staff members must be able to take and pass, without major modifications, a course in Handle with Care. This program requires hands-on, physical components to ensure the safety of the clients and staff. Clinical staff members must possess reliable transportation, ability to acquire an F endorsement on driver's license, and transport clients. HRMC is an Equal Opportunity Employer (EOE). HRMC is currently seeking a qualified and enthusiastic professional for the following position: OnTrack Hamilton County Team Leader. JOB REQUIREMENTS Education/Knowledge: Master's Degree in Human Services or related field with course work or experience in the areas of cultural diversity, human development, etiology and treatment of mental illness, alcohol and drug abuse, physical and sexual abuse, suicide, and developmental disabilities. License preferred. Experience: Master's degree in Human Services or related field. Two-five years experience in the social work field or a behavioral health setting preferred. Skills: Knowledge of etiology and treatment of mental illness, physical and sexual abuse, suicide, human development, and cultural diversity. Physical: Exposure to biological hazards. Hearing of normal and soft tones. Close eye work. Valid driver's license- "F" endorsement required for the transportation of clients. Lifting up to 50 lbs. Pushing/pulling up to 150 lbs. frequent sitting, standing, walking, bending, stooping, and reaching. Required to be certified in, and effectively implement, verbal and physical de-escalation techniques that include a wide range of bodily movements; including but not limited to, grasping, holding another person, going down on knees, running, and walking. An essential job function is a reliable, predictable 40 hour per week job, on site and in the community, with regular attendance. Ability to effectively and ethically counsel youth and young adults and families. Ability to present professionally and work within a team format to plan, implement, and evaluate successful interventions. Ability to effectively run and process groups. Ability to work within a team format to meet positive goals for children and adults and interface with other agencies involved in the ecology of the individual. Must be capable of assisting in utilizing nonviolent methods of crisis intervention including therapeutic holding. Ability to exercise sound judgment and effective decision-making, ability to set and demonstrate appropriate boundaries, ability to be an empathic listener, flexibility, willingness, and adaptability to working with diverse populations. Must also have the ability to communicate effectively and possess good time management and organizational skills. Required to be certified in and adequately implement verbal and physical descalation techniques that include a wide range of bodily movements including but not limited to grasping, holding another person, running, and walking. Physical descalation techniques will only be implemented at Helen Ross McNabb Center facilities, and not in community settings. JOB SUMMARY Serves as the secondary clinician that provides individual, group, and family counseling as part of the First Episode Psychosis/On Track TN model in Hamilton County. Clinician will also serve as the team leader and clinical supervisor for the OnTrack Hamilton TN team, and facilitate the coordinated specialty care team. Team leader ensures weekly individual, group, or family psycho-therapy to adolescents and young adults ages 15-30 with a mental health diagnosis that involves symptoms evident of psychosis. Team leader works in a team format to meet the individual needs of clients in the program, while providing clinical recommendations for treatment. Team leader will help the team and clients clarify goals, cope with stressful situations, interact more effectively with other people, and generally to overcome barriers to client's recovery. Team leader is to ensure psycho-social assessments for clients and diagnosis and appropriate referrals for clients, as needed. Team leader provides crisis intervention and emergency services as needed. The Team leader will rotate in a 24 hour/7 day a week on-call schedule. Ensure treatment modalities used by Master's level clinicians providing therapy services include Dialectical Behavior Therapy (DBT), Cognitive Behavior Therapy (CBT), CBT for psychosis (CBTp), Trauma-Focused Cognitive Behavior Therapy (TF-CBT), Attachment, Self-Regulation and Competency (ARC). Functions as a member of a treatment team to plan, implement, and evaluate successful interventions for individuals and families. Team leader embraces the key values of clinical services: empowerment, normalization, rehabilitation, and continuity of care. Provides administrative duties for the OnTrack Hamilton TN program including supervisory responsibilities, training of new staff members and continued staff development, monthly reporting in house and to the state of Tennessee (TDMHSA), and the promotion of the program with outside agencies. Conducts administrative and clinical supervision to a coordinated specialty care team. In the performance of their respective task and duties, all employees are expected to conform to the following: a. Perform quality work within deadlines with or without direct supervision. b. Interact professionally with other employees, customer and vendors. c. Work effectively as a team contributor on all assignments. d. Work independently while understanding the necessity for communicating and coordinating work efforts with other employees and organizations. e. Conducts individual, family, and group counseling f. Is responsible for maintaining a designated amount of direct service time with clients and a designated amount of contacts for the assigned caseload. g. Participates in the initial individual program planning process with all service providers (i.e., Outreach/Education specialist, SEES, and medical staff) and provides ongoing information regarding client progress to the service providers. h. Completes an individual treatment plan with each client within 30 days of enrollment. i. Writes progress notes after each service contact with a client in accordance with the Center's procedure on Time Frames for Clinical Records and Related Documentation. j. Maintains documentation of a six month service update in the clinical record. k. Participates in the development of a crisis/safety plan for priority clients. l. Participates in regularly scheduled Peer Review. m. Follows all guidelines outlined in the HRMC Policies and Procedures Manual including Corporate Compliance Policies. n. Is expected to have regular and predictable attendance and the ability to work cooperatively with others. o. If licensed, signing the non-licensed clinicians' treatment plans and being available for clinical consult p. coordinates and schedules ongoing outreach/referral meetings with community providers as part of the OnTrack model. Job Duties Quality Care 1. Provides individual/family psycho-therapy for youth and young adults ages 15-30. a. Clinician is present and ready to begin session at the appointment time, every session. b. Clinician calls to reschedule any sessions that he/she will be unable to attend or that the consumer missed or canceled 100% of the time. c. Clinician communicates accurate and thorough information to outside agencies 100% of the time when clients need different or more appropriate care that HRMC cannot provide. d. Ensures therapy contacts are completed each month, according to program standards. Provides diagnostic Psycho-Social Assessments a. Clinician completes intakes and assessments within 5 business days of contact. b. Assessments will be completed in coordination with Assessment Team and Client Benefits as evidenced by communicating any issues concerning clients or assessment. Documentation requirements Ensures all family and client participation in treatment planning as evidence by signatures on tx plans. Communicates to supervisor if referrals become low in weekly supervisions. Meet weekly with direct supervisor to discuss clinical and administrative concerns/ changes Create and implement treatment interventions that are reflected in progress note documentation based on the treatment plan goals Respond to all flags and e-mails within 2 business days Respond to all voice mails within 30 minutes Complete all required clinical documentation in accordance with Center Policy and Procedure and funding source guidelines. Update treatment plans, crisis plans, DLA-20 and updated admissions signature page every 6 months Complete and submit productivity summaries to supervisor by the 3rd business day of the month. Create daily progress notes and sign in EMR within one week of service provided Leadership Provides administrative/clinical supervision to OnTrack TN team Complete and submit written documentation of verbal and written corrections pertaining to staff as needed Complete current annual performances appraisals for OnTrack TN staff Meet with supervisees on a weekly basis for clinical/admin supervision Attends and is timely to weekly multi-disciplinary team meetings Develops and implements coverage for clients when clinicians are out on leave. If licensed, signing the non-licensed clinicians' treatment plans and being available for clinical consult Fill vacancies with qualified candidates Plan and facilitate weekly staff meetings with team. Ensures update treatment plans, consumer strength assessments, crisis plans, consent to contact, releases, ROI, MIRECC and updated admissions signature page every 6 months as evidence by quarterly quality record reviews for OnTrack TN clients Implements a plan to ensure tracking system for Tx plans for all supervisees Ensures all required data for funding is tracked monthly and quarterly, as requested by supervisor. Develop and implement an ongoing plan for community outreach to educate and connect others to the FEP model of treatment. Attend trainings to advance skills in the FEP model, and ensure cross-training of all appropriate roles, as part of the Coordinated Specialty Care (CSC) model.
