Registered Nurse (RN) | Case Manager Location: Atlanta, GA Agency: Lancesoft Inc Pay: $3,050 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: ASAP About the Position TravelNurseSource is working with Lancesoft Inc to find a qualified Case Manager RN in Atlanta, Georgia, 30301! ? Shift Details Shift Time: 08:30 AM – 05:00 PM Shift Length: 8 hours Schedule: Weekly , standard shift Additional Notes: Weekend work may be required Holiday scheduling may also be required ? Key Responsibilities Manage complex cases involving chronic illnesses such as: Diabetes Cardiovascular conditions Behavioral health issues Conduct utilization management (UM) and coordinate care plans. Collaborate with healthcare providers, internal teams, and external partners to ensure quality, cost?effective care. Support patient education, care transitions, and resource utilization. Requirements Experience 5 years of experience in Case Management and/or Utilization Management (UM) Experience managing complex cases , including chronic and high?risk health conditions Strong background in: Diabetes care Cardiac/cardiovascular conditions Behavioral health case management Skills / Competencies Ability to handle complex case loads Strong understanding of care coordination , patient education , and care transitions Knowledge of chronic disease management frameworks Good communication and collaboration skills Ability to work weekends and holidays if needed Certifications BLS (Basic Life Support) – required RN License – active and valid Case Management certification (e.g., CCM , ACM ) is preferred but not required Credentials Modified credentialing for ASAP start 29261852EXPTEMP
