                • Promoting and monitoring effective utilization of health care resources.
                • Assuming a leadership role with the interdisciplinary care team.
                • Facilitating the care of patients during a disease/catastrophic episode by managing, coordinating and monitoring resources to achieve optimal clinical, utilization, service and financial outcomes.
                • Assessing and facilitating appropriate hospital admissions using evidence based criteria.
                • Managing the collaborative process of assessment, planning, facilitation and advocacy for options to meet an individual's health needs.
                • Using Transitions of Care (TOC) to improve the quality of care coordination and communication when patients are transferred from one level of care to another, either internally or externally.
                • On Call may be required (East Campus).The 128-bed Parkridge East Hospital in East Ridge provides a full range of medical and surgical services, such as orthopedics and robotic surgery. Our 24/7 Emergency Department is recognized by the Society of Chest Pain Centers as an Accredited Chest Pain Center. - The Birth Place at Parkridge East offers a comfortable, private setting in which families welcome new additions.                   • Education: Graduate of an Accredited RN program.
                  • Licensure/Certification: -Registered Nurse (RN) license required or RN Compact State Licensure in the State of Residence. Case Management Certification (CCM) or American Case Management Association (ACMA) certification preferred.
