  • You will be facilitating the care of patients during a disease/catastrophic episode by managing, coordinating and monitoring resources to achieve optimal clinical, utilization, service and financial outcomes.
  • You will manage the collaborative process of assessment, planning, facilitation and advocacy for options to meet an individual's health needs.
  • You will use the transitions of Care (TOC) to improve the quality of care coordination and communication when patients are transferred from one level of care to another, either internally or externally.
  • You will complete psychosocial assessments with patient and family Creates aftercare / discharge plan and summary.

    • Graduate of a Master's Degree Program in Social Work.
    • State of TN LCSW
