The RN delivers routine ACT treatment services such as wellness education, coordinating with medical providers, and medication managment (including giving IM medications).This is a field-based position and requires at least 75% of services tobeprovided to participants in their homes, shelters, places of employment, vocational and education settings, and other community settings.

  • Act as part of a multidisciplinary team designed to provide services from a holistic and Values based perspective
  • Manage the ordering/delivery/storage/ administration of medications for the persons served in accordance with the specific program/service
  • Provide education to both persons served and staff regarding common health concerns. Provide training to staff as required/requested in order to best meet the needs of persons served.
  • Assess and respond to new/potential/ ongoing health issues in persons served.
  • Complete documentation as required by RHD, funder/licensing/accreditation bodies.Required ExperienceMinimum of 1 year related experience.Required Education A graduate of a school of professional Nursing (AD, hospital, or BSN); BSN preferredRequired Qualifications
    • Licensed to Practice in Tennessee
    • Valid Driver's LicenseProgram SummaryThe RHD Chatanooga ACT Team provides non-traditional community based mental health treatment to promote recovery and community integration of up to 100 adults with serious mental illnesses and co-occuring substance use disorders living in the community to reduce the use of ER's, inpatient hospital stays, and incarcerations.Resources for Human Development, is an equal opportunity employer.
