  • Asa Case Manager, you will deliver the successful operation of the CaseManagement function.
  • Youwill act as a role model for care delivery and demonstrate WE CARE Valuesin all actions.

    Qualifications
    • Graduate of an accredited school of Nursing -RNor MSW or Equivalent Masters prepared individual.
    • Minimum of 2 years case management experiencepreferred
    • Critical thinking skills, the ability to workwith minimal supervision, assertive and persuasive communication skills.
