Transition Carceral Program JOB SUMMARY: The Community Outreach
Worker works with Transitions Clinic Network (TCN) program
participants to identify, locate, and obtain health, social, and
community services. Additionally, the Peer Recovery Navigator will
identify, enroll, and provide basic workforce development services
to the Transitions Clinic Network (TCN) program participants. The
Peer Recovery Navigator position is responsible for the
coordination of services and needs identified by client; this is a
participant centered service delivery model. This position will
also include facilitation of workshops for small groups of
individuals, will work with justice involved individuals, and
provide one-on-one services for participants to reduce barriers to
employment, recovery, and economic self-sufficiency. This position
will work closely with the Mental Health Case Managers and Ryan
White Case Managers to provide services to participants and support
Transitions Clinic Network (TCN) program activities. JOB FUNCTIONS:
1. Uses own lived experiences with incarceration and/or substance
use recovery as a vehicle to establish rapport and build
relationships with the individuals served. 2. Offers insight into
the experience of living with a substance use disorder and/or other
lived experiences (such as incarceration, homelessness, etc.) and
relevant supports in order to assist program and partners in
identifying program-specific services and linkages that are
conducive to recovery and workforce engagement. 3. Determines
customer eligibility for a variety of workforce development
programs. Assists customers in navigating to appropriate programs,
agencies, and/or resources to support participant economic
self-sufficiency and recovery. 4. Assesses customer need in
partnership with the customer, obtains authorization, and
facilitates supportive services to assist in removing barriers that
may prevent successful completion of the program. 5. Provides
pre-employment and employment services to participants, including
justice involved participants and working inside correctional
facilities with inmates. 6. Monitors and records participant data
into designated case file and information management systems as
needed. TITLE: Transitions Clinic Network (TCN) PEER NAVIGATOR 1.
May facilitate group processes, including testing, workshops,
orientations, and job clubs. 2. Engages with contractors, outside
partners, and community organizations to align and coordinate
services and resources. 3. Maintains strong working relationships
with external organizations such as county jails, community courts,
community-based service agencies, and others. 4. Attends
appropriate team meetings. 5. Pick up case load responsibilities as
needed to cover absences or staff shortages. REQUIRED KNOWLEDGE,
SKILLS AND ABILITIES: 1. Be certified as a Peer Recovery
Counselor/Certified Peer Counselor within Georgia. 2. Experience in
workforce development programs, services, techniques, and
performance, or experience with other community-based service
delivery systems and ability to learn quickly. 3. Excellent
customer service skills. 4. Experience with customer assessment,
program eligibility, and providing support and training services.
5. Ability to understand, interpret and follow federal, state and
local policies and regulations. 6. Strong verbal and written
communication skills. 7. Able to work independently in off-site
locations. 8. Must be organized and possess multitasking skills. 9.
Ability to facilitate training for diverse populations while
maintaining confidence and order. 10. Ability to work with diverse
populations, including the competence and skill to establish
supportive trusting relationships and respect the rights of each
participant on their case load at all times. 11. Able to maintain
high levels of confidentiality, credibility and professionalism.
12. Proven experience with Microsoft Excel, Word and Outlook.
WORKING CONDITIONS/PHYSICAL REQUIREMENTS: The work is performed in
an office environment. Minimal bending, stooping, kneeling or
lifting involved. Position may require travel within workforce
delivery area boundaries. OTHER: This position may require working
hours outside of standard 9am-5pm business hours. This position may
require an enhanced background check for access into criminal
justice facilities.
